Waiver and Permission for 
 Middle - High School Boys Youth Lock in
March 13th-14th

Parents of CCF middle school &  High School boys
         Beginning on Friday  March 13th @ 6pm our rising high school students -Graduating Seniors are invited to participate in our lock in. Students will spend the Night at CCF and wake up on Saturday  Morning  on March 14th for CCF Men’s Breakfast . Students must depart by 10:30 am on Saturday morning 
This will be a time for fellowship & fun for our middle and high school boys, as we will be discussing what it is to be a man after the Lord’s heart
       If parents desire to pick up their students early on Friday night or  please coordinate  with Pastor Joseph.
     Parents we encourage you to help provide snacks & drinks for our time together. Please reach out if you have any questions or would like to know how you can help.

By submitting this form I consent to the Child(ren) above to participate in the CCF Lock in that will be facilitated by CCF staff and/or volunteers. I HEREBY RELEASE, DISCHARGE AND COVENANT NOT TO SUE CAPITAL CHRISTIAN FELLOWSHIP it’s volunteers, and/ or participants FROM ALL LIABILITY TO THE CHILD(REN) NAMED ABOVE, FOR ANY AND ALL CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON ACCOUNT OF INJURY TO THE CHILD(REN) named above, including, but not limited to, death or damage to property, Caused or alleged to be caused , in whole or in part, by the negligence of the child named above or otherwise. .
