Intern Support Team Gathering

Dear Parents,
     On  Saturday, January 25, 2025, Sarah Giordano's Intern Support Team, of which your child is a member, will be having an in-person gathering at Hub 450 in Lancaster, PA (450 N Prince Street, Lancaster, PA) from 5pm-7pm.
Transportation
       Sarah Giordano, along with another adult volunteer, will be providing transportation, departing from Lanham, MD at 3pm and returning at approximately 9pm. If you have any questions, Sarah can be reached at sarahgiordano@capitalchristian.org

By submitting this form I consent to the Child(ren) above to participate in Sarah's Intern Support Team gathering with transportation by CCF staff and/or volunteers to  Hub 450 (450 N Prince Street, Lancaster PA  I HEREBY RELEASE, DISCHARGE AND COVENANT NOT TO SUE CAPITAL CHRISTIAN FELLOWSHIP it’s volunteers, and/ or participants FROM ALL LIABILITY TO THE CHILD(REN) NAMED ABOVE, FOR ANY AND ALL CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON ACCOUNT OF INJURY TO THE CHILD(REN) named above, including, but not limited to, death or damage to property, Caused or alleged to be caused , in whole or in part, by the negligence of the child named above or otherwise.
